    require, is the most common idea behind "something means this", as in "A cavity means a filling". It can also mean slightly less required, something that is only likely not obligatory.  An old car means repairs (you ought to expect repairs will be needed, and it will be just good luck if they aren't).

    Means is used to indicate "equals", or perhaps "says", in a way, when used in that sentence form.  This says that.  If you have this symptom, the interpretation is that problem. A runny nose says cold or allergy. A runny nose means a cold or allergy.  A runny nose is a symptom of a cold or allergy.  A runny nose means antihistamines.  You can get rid of that runny nose by taking antihistamines, perhaps.  A broken leg means a hospital stay.
