In that game can you attack and kill civilians like in GTA games or do they just run away?

You can't hurt civilians at all in Yakuza. You can only attack during a fight. The Yakuza games are more like RPGs then open world action games like GTA. But they're RPGs with action combat and fun open world activities like playing old games in the arcade or fishing or watching porn videos.
