Can you be arrested for telling a cop, who followed you into fast-food drive/thru for not wearing seat belt, “I lost my appetite”?
Let’s say a cop asks for his license/registration and says, “You can pull up to drive-thru window to pay for food and pick it up, and then pull up behind squad car to wait for your ticket. So he says, “I lost my appetite.”

1. NONE of your scenario will EVER happen.
2. You wouldn't be arrested for saying your lost your appetite, but the ATTITUDE you exhibit would tend to prompt the officer to exercise whatever discretion they have AGAINST you.

A ticket is a form of arrest. A ticket means it's a minor charge for which you COULD be arrested, but it's not worth the time and trouble.
That's why if you don't appear a warrant is issued for your arrest.

He'd probably just shrug his shoulders and say "Here's your ticket, enjoy your meal" and leave it at that.
