How come students are not allowed to call a teacher by their first name? How did it become a decision that it is a total insult?

It is disrespectful.  You - a student is NOT on par with their teacher.  You are the student and your teacher is teaching you, who is in a position of authority.  That is why you owe the teacher respect.

Different schools have different rules, and even within a school, different teachers may allow the higher grades to address them by their first name.

But in general it's always safer to address a teacher as Mr, Mrs, Miss or Ms Smith until they tell you different. Using the first name of someone demonstrably your senior can seem very rude.

Your teachers deserve respect, you show that respect by using an honorific.

Yeah but it depends on the teacher. Yes it is a sign of respect but a lot of teachers are a s s holes.
