    Generally, yes, but, if it is not capitalized in a title of a book, it shouldn't be.

    No. It is a very small word. Usually two-letter words are only capitalized if they are verbs or infinitives, such as 'Is' and 'To Be'. The exception would be if it is the first word of a title, such as "As the World Turns."
