Bad breath is one of the symptoms you notice (along with gas, diarrhea, fever, fatigue, mood swings and others) during a suspected parasitic infection. Often these are 'intestinal parasites' caused by improper diet or partially digested food -- which give off the foul smell from the stomach, leading to bad breath in the mouth.
Parasite or worm infestation can come from a number of sources: Walking barefoot in contaminated soil; eating unclean fruits and vegetables, raw meat, raw fish (sushi), playing with pets, working in the garden, mucus-forming foods, lack of proper bowel movement or weak digestive enzymes.
Simple changes to your diet along with oral / stomach / colon hygiene are all you need to do to cure bad breath caused by parasites. It's the parasites that are the root cause here; bad breath is simply one of the symptoms you need to tackle holistically.

I have yet to see a case of parasites intraorally. Nor do I know of any parasites which would do that. If you have not seen a dentist once than the last six months for a regular checkup, you should definitely do that. They can make recommendations regarding bad breath. Also, it is very possible to have cavities which cause bad breath and if you have these, it would be better to address them sooner than later for your health and for your safety. Take care.
